In this role, you will work directly with clients to assess needs, develop personalized service plans, and connect them with vital community resources, housing opportunities, healthcare services, and other supports that promote self-sufficiency and quality of life. Key ResponsibilitiesAssessment & Service Planning Conduct intake interviews and functional assessments to determine eligibility for services. Identify client strengths, needs, and independent living goals. Develop, implement, and monitor individualized service plans. Resource Coordination & Navigation Connect clients with community resources, including transportation, healthcare, vocational rehabilitation, and public assistance programs. Coordinate referrals and maintain relationships with service providers and community partners. Independent Living Skills Development Provide guidance, training, and support in daily living skills, including budgeting, household management, meal planning, and personal organization. Encourage self-advocacy and informed decision-making. Advocacy & Client Support Advocate for client rights and access to services. Assist clients in navigating public benefits and support systems. Provide crisis intervention and conflict resolution when needed. Documentation & Compliance Maintain accurate, confidential client records and case documentation. Track client progress and outcomes. Complete reports and documentation required by funding sources and regulatory agencies. Qualifications •Experience in case management, independent living services, social services, disability services, or a related field preferred. Knowledge of disability rights, aging services, community resources, and public assistance programs. Strong organizational skills with the ability to manage multiple cases and priorities. Skills & Competencies Excellent interpersonal, verbal, and written communication skills. Strong empathy, active listening, and problem-solving abilities. Ability to build trust and maintain professional relationships with diverse populations. Effective time management and attention to detail.
