Coordinates receipt of initial and ongoing clinical, demographic, and financial data for services and referrals and communicates with third party payors and case managers relative to patient coverage and home health benefit packages.

Essential Functions:
Coordinates the referral process for homecare services by receiving and reviewing referrals from healthcare providers, patients, and family members. Communicates with patients and their families to assess their needs and preferences for homecare services. Collaborates with healthcare providers to ensure that patients receive appropriate homecare services. Schedules homecare services and coordinates with healthcare providers to ensure that patients receive timely and appropriate care. Maintains accurate and up-to-date records of patient information, including medical history, medications, and treatment plans. Provides ongoing support to patients and their families, answering questions and addressing concerns about homecare services.
Education Requirement:
Associate's Degree in medical, office, or business fields, or equivalent experience, required.

Knowledge of Medicaid, HMO, insurance regulations, and managed care as they affect home care agencies, preferred. Detail oriented and strong organizational and multitasking skills. Effective interpersonal and customer service skills.
