Full-Time Overview Grafton Oaks is seeking a strong, results-driven Licensed Nursing Home Administrator (LNHA) to lead our skilled nursing facility. This leadership role is responsible for overall facility operations including clinical outcomes, financial performance, staffing, regulatory compliance, and census development. We are looking for an experienced healthcare leader who can drive survey readiness, improve quality measures, and build a strong team culture focused on accountability and resident-centered care. Responsibilities Oversee daily operations of a skilled nursing facility (clinical, administrative, and support services) Ensure compliance with all CMS, federal, state, and local regulations Lead survey preparedness and maintain strong deficiency-free outcomes Manage facility budget, financial performance, and operational efficiency Oversee staffing, recruitment, retention, and labor management (including agency control) Partner closely with Director of Nursing (DON) to ensure quality clinical outcomes Monitor and improve quality measures, QAPI initiatives, and resident satisfaction Develop leadership team and support employee performance management Drive census growth, admissions, and referral relationships Maintain strong communication with residents, families, staff, and corporate leadership Qualifications Active LNHA license in the state of Ohio (required) Minimum 2+ years of experience as a Nursing Home Administrator or Assistant Administrator in skilled nursing Strong knowledge of CMS regulations and long-term care operations Proven experience with staffing management, budgeting, and survey success Strong leadership, communication, and organizational skills Ability to improve clinical and operational outcomes in a fast-paced environment Preferred Experience Multi-facility or corporate healthcare experience History of improving CMS star ratings or survey results Strong turnaround or operational improvement background Benefits Competitive salary based on experience Health, dental, and vision insurance Paid time off (PTO) and holidays Performance-based incentives (if applicable) Supportive regional leadership structure Career growth opportunities within a growing healthcare organization Why Join Grafton Oaks?
