Job Description

Job Overview We are seeking a dedicated and compassionate Case Manager/Independent Living Coordinator to join our team. In this vital role, you will empower individuals, particularly children and youth in residential mental health care. Your expertise will guide clients through complex systems, coordinate services, and foster a supportive environment that promotes growth, safety, and well-being. This position offers an exciting opportunity to make a meaningful difference in the lives of vulnerable populations while working within a dynamic and collaborative team. Responsibilities Develop personalized care plans for clients, focusing on independent living skills, safety, and overall well-being. Coordinate services across multiple agencies, including child protective services, social work teams, healthcare providers, and educational institutions. Provide direct support to children and families involved in child welfare or adoption cases, ensuring their needs are met with sensitivity and professionalism. Monitor client progress through regular check-ins, documentation, and reporting to ensure goals are being met effectively. Advocate for clients' rights and needs within various systems while maintaining compliance with legal and organizational policies. Qualifications Proven experience working with children and adolescent case management. Knowledge of child welfare systems. Strong understanding of childcare practices and family support strategies. Excellent communication skills with the ability to build trustful relationships with clients from diverse backgrounds. Excellent ability to provide timely and accurate written reports and progress notes. Ability to manage multiple cases simultaneously while maintaining attention to detail and confidentiality. Relevant certifications or training preferred. Compassionate approach combined with proactive problem-solving skills to support clients effectively.
