Job Description

Overview The Care Manager plays a pivotal role in coordinating and overseeing patient care within a healthcare setting. This position requires a compassionate and knowledgeable individual who can effectively manage patient assessments, discharge planning, and care transitions. The Care Manager will work closely with healthcare teams to ensure that patients receive the highest quality of care while navigating through various levels of medical services. Responsibilities Coordinate patient care across multiple settings including inpatient, outpatient, and post-acute care. Conduct comprehensive patient assessments to determine needs and develop individualized care plans. Collaborate with multidisciplinary teams to facilitate effective discharge planning and ensure continuity of care. Maintain accurate medical records and documentation in compliance with HIPAA regulations. Provide education and support to patients and families regarding treatment plans, resources, and available services. Engage in utilization management to optimize patient outcomes while managing healthcare resources effectively. Conduct diagnostic evaluations and case management for patients with complex medical needs. Advocate for patients' rights and ensure their needs are met throughout their healthcare journey. Skills Hospital experience, particularly in Level I or Level II trauma centers. Proficiency in acute care settings, including ICU, emergency medicine, and hospital medicine. Knowledge of pediatrics, behavioral health, mental health counseling, and addiction counseling is advantageous. Familiarity with medical terminology, physiology knowledge, and patient assessment techniques. Experience in discharge planning, intake processes, and managing medical records. Strong skills in case management and utilization management within both inpatient and outpatient environments. Excellent communication skills to provide effective patient service and support throughout the care continuum. Ability to work collaboratively with healthcare professionals across various specialties including social work and clinic experience. The Care Manager position is essential for enhancing patient experiences while ensuring high standards of care are consistently met. Candidates should be prepared to engage actively with patients, families, and healthcare teams to foster positive health outcomes.
