Care Manager Mintex Technologies New York, NY Job Details Full-time $50,000 - $60,000 a year 1 day ago Qualifications Working with individuals with developmental disabilities RN License HIPAA Master's degree Bachelor's degree Full Job Description Job Overview The Care Manager is responsible for delivering person-centered care coordination services to individuals with developmental and behavioral health needs. This role supports members through comprehensive care management, service coordination, referrals, transitional care, family support, and community resource linkage while ensuring compliance with OPWDD standards and state regulations. Responsibilities Conduct comprehensive assessments and develop individualized Life Plans Coordinate medical, behavioral, social, and community-based services Maintain regular in-person and virtual member contact based on individual needs Support individuals during transitions of care, hospital discharge, and crisis situations Collaborate with interdisciplinary teams, families, and service providers Monitor member benefits including Medicaid, SSI, and Food Stamps Maintain accurate documentation in Electronic Health Records (EHR) and ensure HIPAA compliance Report incidents of abuse, neglect, or maltreatment promptly Provide referrals and follow-up for community and social support services Qualifications Bachelor's degree with 2 years of relevant experience, OR Registered Nurse (RN) license with 2 years of relevant experience, OR Master's degree with 1 year of relevant experience MSC Service Coordinators prior to July 1, 2018 are grandfathered Skills & Competencies Strong interpersonal, communication, and conflict-resolution skills Knowledge of de-escalation techniques Ability to manage caseloads and prioritize effectively Proficiency in Microsoft Office and Electronic Health Record systems Strong organizational and problem-solving abilities
