POSITION SUMMARY
The Care Manager (CM) is the primary representative of the CMO for youth and families and providers. The Care Manager is responsible for facilitating quality care management services using the wraparound approach. To facilitate services, the Care Manager helps create a Child Family Team; helps identify youth and family strengths and needs; creates and implements individualized service plans; connects families to available resources; monitors progress toward service goals; coordinates services; and helps youth successfully transition from services. Care Manager is a hybrid position which requires both remote work and in-person meetings in the community (including family's homes) and office.
ESSENTIAL RESPONSIBILITIES
- Uses best practice techniques, aligned with the CMO and wraparound model, values, and principles.
- Consistently provides Medicaid-reimbursable services in-person and in the community; uses video and audio communication appropriately and in accordance with approved training, policy, and practices.
- Helps youth and families identify and develop a Child Family Team of formal and informal supports.
- Facilitates the development of an Individualized Service Plan (ISP) for children and youth using individualized, strength-based interventions consistent with CSOC and CGS Family Partnership values.
- Demonstrates professional and timely communication with youth, family and supports, system partners (ex: visits, phone calls, written communication).
- Actively collaborates with service providers, child and family team members, and system partners on plan development and delivery.
- Completes documentation and performs administrative duties such as sending referrals, writing progress notes, completing plans and reports, collaterals, and obtaining signed paperwork in a thorough, timely manner, meeting internal and CSOC expectations.
- Identifies and connects youth and families to community supports and resources.
- Adheres to the state contract expectation of comprehensive care management in a 24/7 environment. Must demonstrate flexibility of tasks and schedule, based on family needs and preferences. Eligible to participate in the on-call phone rotation (approximately twice per year).
- Consistently and promptly attends, as well as actively and professionally contributes, to all required team, agency, and system partner meetings.
- Utilizes individual supervision, debriefs, trainings, and multidisciplinary meetings to enhance learning, growth, and quality of service.
- Demonstrates professional communication and interactions with all CGS employees and community members at all times.
- Achieves Care Manager Certification according to CSOC guidelines and completes other recommended state and agency trainings as appropriate.
- Perform other duties as assigned.

QUALIFICATIONS

- Must demonstrate evidence of specific training/instruction through one of the two options below:
- Must have a master's degree or higher in a related field (e.g., social work, counseling, psychology, psychiatric nursing, criminal justice, specialeducation); or…
- Bachelor's Degree in a related field (e.g., social work, counseling, psychology, psychiatric nursing, criminal justice, specialeducation) plus at least 1-year related experience in behavioral health, substance use, developmental/intellectual disabilities, special education, criminal justice, or case management.
- Can effectively use required technology to fulfill job duties including but not limited to computers, software applications (i.e., Microsoft Office, Outlook, Zoom, Teams, etc.), electronic health records, phones, etc.
- Must pass all background checks, including Medicaid debarment.
- Regular travel required (average 5%-25% travel per week).
Must be able to regularly participate in in-person meetings in the community, including youth/families' homes. Valid driver's license and adequate driving record required. Must remain insurable on a CGS auto insurance plan.
