Position Summary We are seeking a proactive and empathetic Health Home Care Manager to join our team. This hybrid-remote role is the "heart" of our service delivery, acting as a bridge between families and the complex systems of healthcare and social services. As a Care Manager, you will navigate the healthcare landscape to ensure every child in your caseload receives integrated, high-quality care. The ideal candidate is a master of organization who can balance the clinical rigor of documentation with the human touch required for effective advocacy.
Core Responsibilities Comprehensive Advocacy:
Conduct thorough assessments to identify medical, behavioral, and social needs. Develop and implement Individualized Care Plans (ICPs) tailored to each family's unique goals.
Care Coordination:
Act as the primary point of contact between families, medical providers, schools, and community partners to ensure a "whole-person" approach to care.
Meaningful Engagement:
Conduct monthly in-person visits with clients to monitor progress, address emerging needs, and build a relationship of trust.
Meticulous Documentation:
Maintain high-standard, timely progress notes and records in strict accordance with Health Home program standards and state regulations.
Resource Navigation:
Identify and link families to essential community resources, including housing assistance, food security programs, and support groups.
Compliance & Integrity:
Uphold the highest standards of confidentiality (HIPAA) and agency values in every interaction.
Qualifications Education:
A Bachelor's Degree is required (Social Work, Psychology, or Human Services preferred).
Experience:
Previous experience working with children, families, or within human services is highly preferred.
Communication:
Exceptional written and verbal communication skills; you must be able to translate complex care plans into actionable steps for families.
Tech-Savvy:
Proficiency with digital tools, EHR/documentation systems, and remote collaboration platforms.
Independence:
The ability to work independently in a remote environment while maintaining high productivity and accountability.
Bilingual Advantage:

What We Offer Compensation:
$50,000.00 - $60,000.00 per year (based on caseload).
