Care Manager As a Care Manager at IOA, you'll play a vital role in our clients' lives, offering them the support and resources they need to thrive. Your empathy and expertise will help ensure that they can live independently and with dignity.What You'll Do Comprehensive Assessments:
Conduct thorough assessments of clients' psychosocial, physical, mental health, environmental, and spiritual needs. Develop and initiate personalized Community Living Plans.Client Visits:
Conduct home visits, hospital visits, and escorts to medical appointments as needed. Establish and maintain supportive relationships with clients and their support networks.Service Coordination:
Identify and arrange appropriate community services, leveraging your knowledge of Medicare, Medi-Cal, and other programs. Monitor and evaluate the quality and timeliness of these services.Documentation:
Maintain detailed and timely records of all case management activities, ensuring continuity of care.Education and Advocacy:
Educate clients and their support networks about available resources. Provide crisis intervention, advocacy, and problem-solving support.Team Collaboration:
Work closely with community providers and health care professionals, ensuring effective communication and coordination of care.What We're Looking For Educational Background:
B.A or B.S in Social Worker or related field requiredExperience:
At least one year of experience working with disabled adults and/or older adults. Experience with mental and behavioral health diagnoses and substance abuse disorders is highly desired.Skills:
Exceptional communication and presentation skills. Strong case management skills and experience in the community health care delivery system. Detail-oriented with good problem-solving skills and the ability to prioritize multiple tasks. Bilingual English/Spanish preferredAttributes:

Ability to talk and hear during face-to-face communications.Mobility:
Ability to reach overhead and below the knees, including bending, twisting, pulling, and stooping.Coordination:
Multi-limb and eye-hand coordination.Endurance:
Ability to sit at a desk for prolonged periods while working on computers. Ability to move, lift, carry, push, pull, and place objects weighing more than or equal to 40 pounds without assistance.
