Job Description

Experteer Overview In this role you will support care management and care coordination for members, focusing on behavioral health and substance use needs within an ED diversion program. You will work with a multidisciplinary team to deliver integrated, value-driven care that improves outcomes while controlling costs. The position emphasizes addressing barriers, coordinating services, and engaging high-acuity members who frequently use emergency services. You will collaborate on care planning, outreach, and transitions of care to drive safer, person-centered support.
Compensation / Benefits
  • Assess behavioral health needs to determine care coordination eligibility within regulatory timelines
  • Develop and implement person-centered care plans with the member and care network
  • Conduct telephonic, in-person, or home visits as required
  • Monitor care plan effectiveness, document interventions, and adjust goals as needed
  • Maintain an ongoing member caseload with regular outreach
  • Promote integration of behavioral health with LTSS and community resources
  • Facilitate interdisciplinary care team meetings and ICT collaboration
  • Apply motivational interviewing and clinical guideposts to support change
  • Identify barriers to care and provide coordination assistance
  • Provide consultation and recommendations to peers as needed
  • Travel locally 25-40% as required Tasks
  • At least 2 years in health care, preferably behavioral health
  • Active licensed behavioral health clinician (LCSW, LMSW, APSW, CHES, LPC, LPCC, LMFT, PhD/PsyD or equivalent) with current license
  • Valid driver's license and reliable transportation for travel
  • Experience with severe mental health concerns, substance use disorders, or foster care
  • Knowledge of whole person care, chronic conditions, and discharge planning coordination
  • Data entry experience and proficiency with a clinical platform
  • Excellent verbal and written communication
  • Proficiency with Microsoft Office and applicable software Key requirements
