Care Manager, LTSS (Remote in Idaho) Molina Healthcare Medicine and Health Boise, ID - USA Manager / Team Leader Show compensation Hybrid Apply now Save position Experteer Overview In this role you support Idaho Medicaid as a field-based Care Manager, coordinating long-term services and supports for high-need members. You partner with a multidisciplinary team to ensure progress toward outcomes, delivering quality, cost-conscious care across home and community settings. You'll conduct in-home assessments, drive waiver processes, and develop care plans with members, families, and clinicians. The position blends travel, independent work, and cross-functional collaboration to improve member outcomes and access to services.

competitive benefits and compensation package Responsibilities Complete comprehensive member assessments, including in-person home visits Facilitate waiver enrollment and disenrollment processes Develop and implement care plans with members and care network Monitor and adjust care plans based on progress and barriers Promote integrated services across behavioral health, LTSS, and community resources Assess medical necessity and authorize waiver services Evaluate benefits and funding sources Lead interdisciplinary care team meetings for service decisions Use motivational interviewing to educate and motivate members Identify barriers and coordinate psycho/social, financial, and medical support Identify critical incidents and create prevention plans Collaborate with licensed care managers/leadership as needed Travel 25-40% for member visits in Idaho Key requirements At least 2 years healthcare experience, including 1 year with persons with disabilities/chronic conditions and LTSS; 1 year in care management or related medical/behavioral health setting LPN/LVN license required only if mandated by state contract or licensing rules; if licensed, active/unrestricted Bachelor's degree in a health care field may be required in some states (dependent on state) Valid driver's license and reliable transportation for travel Knowledge of community resources and ability to work across diverse settings Strong time-management, problem-solving, and communication skills Proficiency with Microsoft Office and navigating online portals/databases Ability to work independently and maintain professional relationships Ability to multitask, manage field work, and document contacts accurately Effective communication Empathy and patient-centered approach Time management and prioritization Microsoft Office suite proficiency Ability to navigate online portals and databases Assessment and care-planning tools (implied)
