In this role, you serve as the essential link between various treatment components, ensuring that adult and child clients are supported from their initial intake through their long-term recovery journey. This position utilizes a strength-based model to provide comprehensive care coordination and transition support.
Key Responsibilities Client Engagement:
Conduct timely post-intake follow-up calls to ensure a positive experience and explain the value of care management services.
Assessments & Planning:
Facilitate initial screenings, assist in developing person-centered care plans, and secure necessary consents for service coordination.
Care Coordination:
Act as a liaison between physical and behavioral health providers, helping clients schedule appointments and overcoming barriers to treatment compliance.
Ongoing Support:
Maintain regular weekly contact with clients to reinforce engagement, monitor progress, and ensure services meet the client's identified needs.
Transitional Care:
Facilitate the successful transition of clients between initiative teams and local community centers, ensuring continuity of care.
Holistic Advocacy:
Connect clients with essential community resources such as housing, food, and employment, while providing education on independent living skills.
Requirements Education:
Bachelor's degree in a health-related behavioral science field (Psychology, Social Work, etc.).
Skills:
Strong communication, active listening, and the ability to collaborate effectively in a remote team environment.
Mindset:
A commitment to treating every individual with respect and focusing on the unique strengths of families and individuals.
Pay:
$40,000.00 - $44,000.00 per year
