The Children's Care Manager is responsible for coordinating services, advocating for families, and ensuring children receive the resources and supports necessary to achieve emotional, behavioral, educational, and social success. This position works collaboratively with families, schools, DCS, probation, healthcare providers, and community partners to help children thrive. Essential Responsibilities Conduct assessments and identify strengths, needs, and barriers to care. Develop and monitor individualized service plans. Coordinate behavioral health, medical, educational, and community services. Facilitate Child and Family Team (CFT) meetings. Maintain regular contact with youth and families through home, community, and office visits. Collaborate with schools, DCS, probation officers, healthcare providers, and community partners. Assist families in accessing resources including housing, food assistance, educational services, and healthcare supports. Provide crisis support and linkage to appropriate services. Maintain timely, accurate, and compliant documentation in accordance with AHCCCS, agency, and licensing requirements. Participate in staff meetings, supervision, and professional development opportunities. Advocate for children and families to ensure continuity of care and positive outcomes. Excellent task prioritization skills combined with a compassionate approach to patient care and family support Ability to work effectively with children of diverse backgrounds and those with disabilities or special needs Strong communication skills for collaboration with multidisciplinary teams including social work, behavioral health specialists, and outpatient providers Minimum Qualifications Bachelor's degree in Social Work, Psychology, Counseling, Human Services, Sociology, Family Studies, or a related field. Minimum of one (1) year of experience working with children and families in behavioral health, child welfare, or community-based services. Knowledge of the AHCCCS behavioral health system and community resources. Experience working with children with behavioral, emotional, developmental, or trauma-related challenges preferred. Strong organizational, communication, and documentation skills. Ability to work independently and manage multiple priorities. Two (2) or more years of care management or case management experience. Experience facilitating Child and Family Team (CFT) meetings. Preferred Qualifications Bilingual (English/Spanish) preferred. Requirements Valid Arizona Driver's License and reliable transportation. Current Fingerprint Clearance Card. CPR and First Aid Certification (or ability to obtain upon hire).
