Monday - Friday, 9:00am to 5:00pm (some evening or weekend availability may be required on occasion for urgent client needs) Willingness to travel across Sacramento, El Dorado, Placer, Nevada, Yolo, and San Joaquin Counties. The Opportunity As a Lead Care Manager serving our Medi-Cal members, you will deliver person-centered care management that helps older adults remain safe, supported, and living with dignity. You'll be trained to work across both of our Medi-Cal programs—CalAIM and the Assisted Living Waiver Program—partnering closely with an interdisciplinary team. You will conduct monthly in-person visits, assess needs, update care plans, coordinate services, and connect participants with critical resources. This hybrid role blends field work, office time, and remote charting, offering flexibility, professional autonomy, and a tangible opportunity to impact our community. What You'll Do Manage a caseload of Medi-Cal program participants, ensuring continuity and quality of care Conduct monthly face-to-face visits and follow-up telephonic encounters to monitor service delivery, assess needs, and update care plans Coordinate services and referrals (e.g., IHSS, transportation, housing, food, behavioral health, medical) Maintain timely, accurate progress notes and documentation per program and regulatory guidelines Collaborate with fellow care managers, RNs, and community partners to resolve barriers and close care gaps Advocate for participants and caregivers, escalating risk concerns and addressing social determinants of health Track outcomes and contribute to program improvements and best practices What You'll Bring Bachelor's degree in social work, psychology, counseling, rehabilitation, gerontology, sociology or a related field (required) 2+ years of related experience in care management, casework, or community-based services Valid CA driver's license, reliable vehicle, and proof of insurance; ability to travel regularly within the listed counties (mileage reimbursed at IRS rate) Strong interpersonal skills, cultural humility, and ability to build trust with diverse older adult populations Excellent documentation, organization, and time management in a field-based, hybrid environment Proficiency with computers and willingness to learn internal EHR and documentation systems. Self-directed with the ability to manage a dynamic caseload and prioritize shifting needs. Nice to Have Experience with Medi-Cal, CalAIM, Assisted Living Waiver Program or other community-based programs Familiarity with electronic health/case management systems Bilingual or multilingual capabilities Lived experience relevant to the role (e.g., family caregiving for an older adult, navigating Medi-Cal/aging services, or firsthand experience with barriers faced by underserved communities) Work Environment and Schedule Hybrid role: part-time in-office, field visits, and remote charting Collaborative, supportive team with flexibility and autonomy Requirements and Pre-Employment Screening Must pass a background check and preemployment screening as a condition of employment Equal Opportunity Senior Care Solutions is an equal opportunity employer.
