The Social Care Manager ( SCM ) works collaboratively with patients/caregivers, providers, and the Medical Management team to coordinate care. This coordination emphasizes addressing social determinants of health, conducting comprehensive psychosocial assessments, connecting members to community resources, and partnering with other agencies. The SCM assists patients, families/guardians, and the healthcare team through the care coordination process and demonstrates strong communication and interpersonal skills. Making outbound calls to enrolled members, review, research and respond to questions or issues via email or phone. This is a Monday - Friday role, 8:00 - 5:00 PM MT.
