Job Description
Hope Services, LLC is seeking a Care Manager within our Tailored Care Management Department. Applicant must be a qualified professional (not licensed or provisionally licensed). The primary responsibilities of the Care Manager are: To promote whole-person. Integrated care by planning. Coordinating, tracking. Closing gaps in care monitoring care to individuals, and achieving key goals of the tailored plan related to behavioral health, mental health, and physical health. Must be comfortable with direct patient contact, including in-home and community appointments. Experience with NC Tailored Care Management is required. The Care Manager shall report to the Care Manager Supervisor. This is a hybrid position with some in office.
