In this vital role, you will coordinate comprehensive social work services, including mental health counseling, case management, and crisis intervention, to enhance patient well-being and support recovery. Your expertise will help bridge the gap between medical treatment and emotional support, ensuring patients and their families receive holistic care tailored to their unique needs. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals facing complex health challenges. Duties Conduct thorough patient assessments to identify social, emotional, and behavioral needs, utilizing clinical research methods and diagnostic evaluation tools. Develop personalized care plans incorporating behavioral therapy, motivational interviewing, and psychotherapy techniques to promote positive health outcomes. Provide individual and group counseling sessions focusing on mental health counseling, addiction counseling, dementia care, PTSD care, and working with individuals with developmental disabilities or autism. Coordinate with healthcare teams to facilitate intake processes, medical documentation, and maintain accurate medical records within HIPAA-compliant EMR (Electronic Medical Records) systems. Manage crisis situations through crisis management protocols and intervention strategies, ensuring patient safety in inpatient, outpatient, hospice, assisted living, or home & community care environments. Collaborate with families and caregivers to deliver education on conditions such as Alzheimer's disease or dementia and support behavioral health strategies tailored for senior care or pediatrics. Track data collection metrics related to patient progress and treatment efficacy for clinical research purposes while adhering to
ICD-9/ICD-10
coding standards for documentation accuracy. Requirements Bachelor's or Master's degree in Social Work (MSW preferred), with licensure or certification as required by state law. Proven experience in hospital settings, inpatient units, nursing homes, outpatient clinics, or home & community care environments. Strong knowledge of medical terminology, CPT coding, ICD coding (ICD-9/10), and familiarity with EMR systems for efficient documentation. Expertise in behavioral health modalities such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and clinical counseling techniques. Experience working with diverse populations including children, seniors, individuals with disabilities or autism spectrum disorder (ASD), Alzheimer's disease patients, or those with PTSD. Excellent communication skills with the ability to perform intake assessments and deliver compassionate patient care in high-pressure situations.
