SUMMARY OF WORK PERFORMED
Part-Time/20 Hours. Provide case management services to youth (case load will vary depending on census) involved with Kitsap County area. Help youth with skills to successfully transition to independence.
This includes:
providing transitional support services, housing, education, employment and basic life skills to work toward permanency. Responsibilities include initial intake through exit processes with youth ages 15+, ensuring ILS contract compliance with Kitsap County.
JOB DUTIES AND RESPONSIBILITIES
1. Cultivate contacts and relationships with outside agencies and individuals that have both direct and indirect impact on the youth's independence and transition. These include but are not limited to: a. Probation officers b. Youth support system (Natural Families, guardians, etc.) c. Educational institutions d. Landlords e. State Regional Personnel f. Health and Mental Health Professionals 2. Initiate and complete all documentation including: a. Intake and exit forms b. Learning plans and Girl's Court Reporting c. Quarterly assessments d. Case manager contact records e. Quarterly reports f. All other documentation described in the corresponding contract. 3. Conduct group activities for youth and staff involved 4. Meet with youth monthly as per the contract. 5. Advocate on behalf of the youth with educational institutions, employment agencies, and other community service providers. 6. Regularly communicate with child welfare professionals, court, probation officer, biological parents and others in the youth's support system. 7. Develop and maintain a supportive relationship to help guide youth to independence 8. Participate in regular staffings, team meetings, staff meetings and trainings.
REQUIRED QUALIFICATIONS
1. Bachelor's Degree or Master's Degree from an accredited college or university in social services or related field. 2. Valid Washington State driver's license and acceptable driving record as required by Olive Crest's insurance carrier. 3. Reliable vehicle and current automobile insurance, indicating your name on the coverage. 4. First Aid/CPR certification. 5. Must have 2 hours of HIV/blood borne pathogens training in the past 5 years. 6. Criminal record clearance. 7. Ability to lift 25 lbs. and intervene during crisis situations with minors up to 18 years of age. 8. Adequate vision and hearing to be able to assess clients needs. 9. Must have a philosophy of support for at-risk children and families in line with the mission, vision, values, and statement of faith of Olive Crest.
DESIRED QUALIFICATIONS
1. Master's Degree from an accredited college or university in social services or similar field. 2. Experience working with at-risk adolescent populations. 3. Prior case management experience. 4. Experience in a culturally diverse workplace. 5. Proficient Microsoft Suite computer skills. 6. Advanced oral and written communication skills. 7. Strong interpersonal and organizational skills.
