In this dynamic role, you will serve as a vital advocate, guiding clients through benefits enrollment, coordinating services, and providing personalized support tailored to each individual's needs. Your energetic approach will foster independence, enhance quality of life, and promote community integration for those you serve. This position offers an exciting opportunity to make a meaningful difference while utilizing your skills in social work, behavioral health, and working with people with disabilities. Responsibilities Assist individuals with developmental disabilities and other disabilities in understanding and accessing available benefits, including Medicaid, Social Security, and housing assistance programs. Conduct comprehensive assessments to identify client needs related to daily living activities (ADLs), behavioral health, communication, and community participation. Provide direct support with activities such as behavior management strategies, and skill development for independence. Utilize data collection tools to monitor progress, document service delivery, and evaluate outcomes for continuous improvement. Support clients with communication challenges by employing techniques such as American Sign Language or other augmentative methods. Demonstrate IL Philosophy in daily work action Provide Information and Referral Services Provide Advocacy Services Provide Peer Support Services Provide Skill Training Services Provide Transition Support Services Provide Assistive Technology and Home Modification Services Support Consumers in the Development of Independent Living Plans Receive training necessary to stay well informed on all aspects of benefits for persons with significant disabilities Frequently meet with DVR counselors and consumers to discuss benefits, work incentives, provide ongoing support, and to empower disabled individuals to make informed employment choices. Provide in-depth written analysis of benefits, written in language the consumer will be able to understand. Keep apprised of health/Long Term Care benefits, work incentive programs, employment impact and options. Complete all paper work in a timely and accurate manner. Qualifications • Bachelor's degree in Human Services or other related field preferred An equivalent combination of education and experience in providing support to persons with disabilities may be substituted for the college degree Two or more years of documented experience working with people with disabilities or firsthand experience with a disability required Knowledge of local community resources and ability to develop networks required Two years of documented experience developing and advocating for services to persons with disabilities required Knowledge of the human services systems, resources and related services required Knowledge of disability rights and IL philosophy required Ability to communicate effectively orally and in written form required Knowledge of and ability to use computer and basic office programs (Microsoft Office) required Knowledge of Assistive Technology and related resources preferred Knowledge of Federal, State, and County funding sources preferred Knowledge of accessible and affordable housing preferred Knowledge of Americans with Disabilities Act preferred Valid driver's license with a clean driving record Compassionate attitude combined with organizational skills necessary for coordinating multiple services simultaneously.
