South Haven Health & Rehabilitation is seeking a PRN Registered Nurse (RN) to serve as our Resident Assessment / MDS Coordinator (Resident Assessment Coordinator) . Why Nurses Love This Role Flexible Schedule Day shift hours No routine medication pass Minimal direct resident care outside of assessments Build meaningful relationships with residents and families Utilize your clinical judgment, assessment, and documentation skills every day Position Summary The Resident Assessment / MDS Coordinator leads the interdisciplinary assessment and care planning process for residents. Working closely with nursing, therapy, dietary, social services, and physicians, the MDS Coordinator helps ensure each resident receives individualized, goal-centered care while maintaining compliance with state and federal regulations. Responsibilities Coordinate the resident assessment process and MDS completion Lead interdisciplinary care plan development and updates Ensure comprehensive care plans are completed according to company, state, and federal guidelines Monitor resident progress and identify care plan revisions as needed Collaborate with department leaders to ensure quality outcomes Review clinical documentation to support accurate assessments and reimbursement Serve as a resource and educator regarding MDS and care planning processes Qualifications Current Registered Nurse (RN) license in good standing Strong assessment and documentation skills Excellent critical thinking and analytical abilities Knowledge of nursing standards of practice Computer proficiency and experience with electronic medical records Long-term care or MDS experience preferred, but training is available for the right candidate We are an equal opportunity employer and value diversity at our company.
