Summary:
The PHSO RN Care Manager provides care management services for selected at-risk patients across assigned value-based care (VBC) populations. The RN Care Manager conducts screening/assessment and risk stratification, develops and monitors individualized, patient-centered care plans and goals, and coordinates care across the continuum—including transitions of care—to improve outcomes and reduce avoidable utilization (e.g., ED visits/readmissions). The RN Care Manager identifies gaps and duplication in services, supports patient self-management and adherence to care plans, collaborates with interdisciplinary partners (including Behavioral Health for select populations as assigned), and documents interventions and outcomes according to departmental standards. Performs other duties as assigned.

MINIMUM REQUIREMENTS
Registered Nurse (RN) - active, unencumbered license in the State of Arkansas is required. Bachelor's Degree in Nursing is preferred (or equivalent as applicable). Case management certification (e.g., CCM) recommended within a specified time frame. Previous case management/care coordination experience or related field preferred. Strong interpersonal, communication, and documentation skills; ability to manage workload and prioritize independently. This job will be authorized 80.00 hours bi-weekly.
