N/A Additional Details about the position Provides clinical management to the UC department and Infusion through coordination of patient care activities, procedures and flow of patients between various units. Acts as liaison between UC staff, patients, and other departments to develop and ensure a smooth process for the safe and effective care of patients in the UC and Infusion Collaborates with admitting, both in house and with outside facilities, to facilitate the plan for timely placement of admissions and transfers Coordinates multi-disciplinary patient care conferences for high risk or complex patients as needed

EXPERIENCE REQUIREMENTS 1

year case management, or1 year care coordinator Must have some RPMS charting experience (or Omnicell)

CERTIFICATION REQUIREMENTS BLS ACLS PALS TNCC

(not required until 6 months) Enpc (within a year)

LICENSURE REQUIREMENT

Any State License
