Job Description
LVN Unit Manager π Marysville Post Acute - Marysville, CA π² $35-$45/hour (DOE) Join a Team That Puts Residents First At Marysville Post Acute, we are committed to providing compassionate, high-quality care in a supportive and collaborative environment. We are seeking a dedicated LVN Unit Manager to lead care coordination efforts and ensure our residents receive exceptional, personalized care. What You'll Do Serve as a key point of contact for residents and families regarding care plans, therapies (PT/OT), and treatment updates Coordinate and oversee admissions, discharges, and referrals Participate in daily stand-up meetings to support team communication and continuity of care Monitor resident progress and adjust care plans as conditions change Perform utilization reviews to ensure timely and cost-effective care Ensure accurate, compliant, and timely documentation Communicate effectively with physicians, providers, and insurance/payor sources Support residents and families so they feel informed, safe, and confident in their care What We're Looking For Active LVN/LPN license (RN a plus) Bachelor's Degree in Nursing Minimum 2 years of clinical experience Knowledge of Medicare, Medi-Cal, and Medicaid Experience with PointClickCare (PCC) preferred Strong communication, organization, and time management skills Ability to work independently in a fast-paced environment Team player with a collaborative mindset Why Join Marysville Post Acute?
