In the Care Management Department, the Nurse Manager Care Coordination leads a high-performing transitions-of-care program in a comprehensive, full-service hospital within a large system—rooted in strong case management and a mission-driven, patient-centered culture and supported by high-quality, award-winning care and a Regional Level II Trauma Center.

Reporting to the Director of Care Management, the RN Manager Care Coordination oversees daily operations, interdisciplinary rounds, utilization review, denials prevention, complex discharge planning, LOS/readmission reduction, payer collaboration, quality dashboards, budgeting, staffing, and continuous improvement; this Full-Time Day leadership role partners closely with friendly bedside nurses, physicians, APPs, pharmacy, PT/OT, and social work in a diverse, collaborative team to deliver seamless transitions and measurable results.

To qualify for the RN Manager Case Management role, candidates must hold a Bachelor's in Nursing (BSN), have an active California RN license at hire, and bring 5+ years of healthcare experience including 3+ years in acute care case management and at least 1 year of healthcare leadership; a Master's in Healthcare or a related field and national certification (CCM or ACM) are preferred and strengthen the RN Manager Case Management candidacy.
