As an LVN, you will play a vital role in providing exceptional patient care to our residents, ensuring their health and well-being are supported in a nurturing environment. This position is for
PER DIEM
(12 hours/day, 1-2 shifts/week) and candidates with prior experience in skilled nursing or subacute facility with basic wound care, trach, g-tube, medications and the ability to take charge will be highly valued. Duties Provide direct patient care, including monitoring vital signs, administering medications, and performing treatments as prescribed. Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement care plans for residents. Assist in assessing residents' health conditions and responding to medical emergencies as needed. Document patient information (e.g., progress and responses to treatments) accurately and in compliance with facility protocols. Educate and support residents and families on healthcare needs, treatment plans, and post-discharge instructions. Maintain a clean, safe, and compassionate environment for residents. Ensure narcotic records are accurate and up to date for your shift. Review medications and confirm the completeness of information. Accurately transcribe physician's orders, initiate, and update care plans. Complete and file recordkeeping forms/charts upon the resident's admission, transfer, and/or discharge. Take vital signs and measurements, including height/length and weight, as needed. Contribute to our mission to provide distinctly high-quality care for residents in a respectful and loving manner.
Qualifications:
Current and valid LVN license in the state of California. At least 2-3 years of experience in subcute care, skilled nursing, or long-term care settings. Experience in psychiatric care is a plus. Ability to work effectively under pressure and manage multiple tasks in a fast-paced environment. Excellent communication skills, both verbal and written. Strong attention to detail and commitment to providing high-quality care. Compassionate, patient-focused approach to care.

Night Shift (12 hour night, 1-2 shifts per week)
