Must be able to genuinely care and engage with our prospects, their families and the residents at Town and Country, and be comfortable carrying out our mission: "To demonstrate the care, love, and hope of Jesus Christ to senior adults." Accept assigned duties in a cooperative manner from your supervisor. Answer Manor calls and identify needs of residents in a timely manner. Assess on a weekly basis the skin condition, stage, size (measurements), site, depth, color, drainage and odor of condition. Assume responsibility for compliance with federal, state and local regulations within the assigned unit. Attend staff meetings, in-service classes and committee meetings as assigned. Be responsible for the safety of residents under his/her supervision. Check meal trays to assure proper diet is served and residents are assisted as necessary. Come to work as scheduled and consistently demonstrate dependability and punctuality. Complete accurately weekly summary. Complete documentation related to the resident's discharge/transfer. Complete report for facility committees as assigned. Conduct resident rounds daily, reporting problems to nursing supervisor and initiating corrective action. Consistently work cooperatively with residents, nursing staff, physicians, families, consultant personnel, community agencies and ancillary service providers. Control supplies and equipment within the unit. Counsel nursing assistants and recommend disciplinary action, when necessary, to the Director of Nursing Service. Develop an accurate comprehensive care plan based on each resident's comprehensive assessment. Document comprehensive assessment of each resident. Document daily notes on change of condition, Medicare/skilled resident and recently admitted residents. Document resident care provided and resident's response or lack of response to care and treatment provided. Document weekly progress notes as assigned and review, revise resident care plan when document weekly and daily notes. Follow facility admission procedures and communicate resident admission to other departments, physicians and ancillary service providers. Follow facility discharge/transfer procedure and coordinate discharge/transfer with other departments, physicians and ancillary service providers. Identify resident problems and emergency situations and initiate immediate "life saving" measures in the absence of a physician. Knowledgeably and competently perform basic nursing skills. Listen to resident, family or physician concerns attentively and initiate corrective action and/or report concerns to management. Order medications as needed, according to facility procedures. Prepare and administer medication, enteral nutrition, parenteral nutrition and treatments as ordered by registered nurse Prepare nursing assistants' assignments and explain all pertinent resident care. Process inquiries from the ER or hospital, and prepare for admission after hours. Work cooperatively with all facility department supervisors and administration.
