RN Care Manager, Full-Time

Primary Care Registered Nurse (RN) Care Manager is responsible for the ongoing development, implementation, and evaluation of processes of the CWHS Case Management Plan, the ACO, and Commercial Payors. The Care Management function is a highly collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client's/patient's health and human service needs. The RN Care Manager provides advocacy, communication, collaboration, resource management, and education to interdisciplinary team members. Responsible for assisting with Annual Wellness Visits, Transitions of Care post-ED and hospital, and assessing patient needs (medical, functional, psychological). The RN Care Manager will have a broad knowledge of nursing practice, the ability to multi-task, and excellent time management skills to ensure all tasks are completed in a timely fashion.
