Job Description

The Weekend Coordinator is responsible to the Manger of Clinical Operations for supervision of the Agency's service programs and patient care activities on the weekend/after hours. Maintains and promotes quality of patient care, in accordance with the Agency objectives and policies. EEO/AA/Disability/Veteran Responsibilities 1. Manages extended hours patient care activities including scheduling nursing visits, assigning work, coordinating report, participation in patient conferences, and documentation review as necessary. 2. Coordinates all patient care activities after hours, on weekends, and holidays including triage of patient calls and referral coordination. 3. Oversees clerical support on weekends/holidays. 4. Orients Home Health Aides to patient plans of care as needed. 5. Documents pertinent patient information in the EMR. 6. Submits reports as requested and offers recommendations regarding extended hours. 7. Maintains communication with the answering service as necessary. 8. Attends staff meetings, and any other meetings as requested. 9. Completes all required educational requirements. 10. May be requested to assist with additional clinical tasks. Qualifications
EDUCATION BS
in Nursing; MSN, MS in Healthcare Management or Public Health desirable. B.S.N., current Connecticut R.N. license, and three years of full-time clinical experience in nursing, at least one of which was in a home health agency which included care of the sick.EXPERIENCE
Three years of full time clinical nursing experience with two years experience in home health as a community health nurse.LICENSURE
Registered Nurse with Valid (or eligible) Connecticut license required.SPECIAL SKILLS
Excellent oral and written communication. Knowledge of home care practices.PHYSICAL DEMAND
Must be capable of standing, walking, bending. Standing, walking and handling are performed on a constant basis. Sitting, carrying, bending, lifting/lowering, pushing/pulling with whole body and with arms are performed on a frequent basis. Squatting/kneeling, twisting, and reaching are performed on an occasional basis. The highest physical demands of this position are incurred while transferring client requiring maximal assist of one or more persons, for example, side lying to sitting, sitting to standing, and lifting from chair, commode, toilet, or bed. Loads lifted or forces exerted up to 70 pounds for client weighing up to 220 pounds in height range of 15-36 inches. Clear vision. Accurate hearing. YNHHS RequisitionID 174983
Qualifications:
EDUCATION BS
