Job Description

Description:
Manage, direct and coordinate the overall health care services for Lumibility. Ensure services are in compliance with agency policies and all regulatory requirements. Supervise and train all staff. Promote community awareness of agency services.
Hours:
40 hours/week
M-F Exempt Location:
Wallingford This position will be based in Wallingford, CT, however travel will be required to other locations if needed.
ESSENTIAL JOB FUNCTIONS
Direct and coordinate the administration of health care services for Lumibility to ensure compliance with all agency policies and regulatory requirements. Provide overall supervision of assigned staff by ensuring effective hiring, training, scheduling and performance evaluation. Oversee and monitor contracts for nursing services. Serve as Medication Administration Coordinator and oversee the medication administration certification process for the agency. Oversee and facilitate staff training on medical procedures, health-related in-services, and delegation of nursing tasks. Ensure completion of medication audits in compliance with regulatory requirements. Ensure and provide Health Care Coordination as part of the Agency's overall service delivery. Serve as liaison between agency and state nursing services. Attend and participate in staff meetings as needed and required. Ensure and provide nursing availability and on-call coverage in accordance with DDS regulations and requirements. Actively participate in activities that promote agency mission and strategic plan. Serve on agency health, safety and other committees as assigned. Ensure timely completion of all required and requested reports and requests for information. Develop and maintain positive relationships with service participants, families, funders and all other stakeholders. Promote an atmosphere of safe working conditions and practices to ensure the health, safety and well-being of staff and persons served.
Requirements:
Bachelor's degree preferred. High school diploma required. Must have and maintain a current state Registered Nurse (RN) license. Minimum five years of general nursing experience with 2 years supervisory or management experience preferred. Must be able to provide leadership in a team setting utilizing community-based resources. Must be over 18 and possess a valid Connecticut driver's license with an acceptable driving history as defined by Lumibility's policy.

Knowledge of person-centered, outcome-based services for people with differing abilities. Knowledge of company policies and procedures and applicable regulatory requirements and standards. Knowledge of community resources. Skill in supervising and training staff in a positive manner. Skill in planning and organizing. Skill in problem solving and decision making. Skill in working with people with differing abilities. Skill in operating standard office equipment, computer and various software packages. Skill in communicating effectively in a courteous and professional manner. Skill in writing clearly and concisely to maintain accurate records. Ability to read and understand policies and regulations and to translate general guidelines into specific action. Ability to attend to the physical and personal needs of adults with differing abilities. Ability to demonstrate good communication and interpersonal skills necessary to provide guidance and instruction to adults with significant challenges. Ability to work successfully, both independently and in a team environment. Ability to maintain confidentiality. Ability to use adaptive and other equipment.
