RN Care Manager (PACE Program) 📍 Wilmington, NC About the Role We are seeking a compassionate and experienced Registered Nurse (RN) Care Manager to join our team. In this role, you will coordinate and deliver high-quality care for older adults, working closely with an interdisciplinary care team to support participants' health, independence, and overall well-being. Key Responsibilities Coordinate care with physicians, social workers, and interdisciplinary team members Perform assessments, triage, and ongoing monitoring of participants Develop and manage individualized care plans Provide skilled nursing care in clinic and home settings Educate participants, caregivers, and families Oversee medication management and chronic condition monitoring Ensure accurate documentation and regulatory compliance Support quality improvement and care coordination initiatives Qualifications Active Registered Nurse (RN) license (NC) Graduate of an accredited nursing program (BSN preferred) Minimum 2 years RN experience Experience working with elderly or frail populations required Home care experience preferred CPR certification (or willingness to obtain) Valid driver's license and reliable transportation What We're Looking For Strong clinical judgment and critical thinking skills Excellent communication and teamwork abilities Ability to work independently and manage multiple priorities Passion for providing person-centered care Why Join Us?
