Clinical Manager RN Home Health Full-Time | Competitive Pay + Benefits + Career Growth

We are seeking an experienced and compassionate RN Clinical Manager with a strong background in home health to join our leadership team. This individual will play a pivotal role in overseeing clinical operations, ensuring the highest quality of patient care and maintaining regulatory compliance within our home health agency. The RN Clinical Manager will serve as a mentor and leader to the clinical staff, driving performance, patient outcomes, and operational excellence. The Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel: Develops, plans, implements, analyzes, and organizes clinical operations. Supervise and support clinical staff including RNs, LPNs, therapists, aides, and other home health professionals. Review and approve patient assessments, care plans, and documentation. Conduct routine case conferences and participate in interdisciplinary team meetings. Monitor clinical performance, caseloads, and patient outcomes. Provide field supervision and performance evaluations as needed. Collaborate with the intake, scheduling, and administrative teams to ensure efficient operations. Assist in hiring, onboarding, and training new clinical staff. Participate in quality improvement initiatives and ensure documentation accuracy. Serve as a liaison between clinical staff, physicians, patients, and families. Required Experience/Skills Graduate of an accredited School of Nursing. Registered Nurse (RN) license in good standing, required. Clinical leadership experience in a Medicare-certified home health agency. Strong understanding of PDGM, QAPI and care coordination. Passion for delivering high-quality patient-centered care in a home setting. Must know OASIS and two years as a Registered Nurse with at least two year of management experience in home care.
