Job Description
The Clinical Care Coordinator will supervise the LPN's and Health Services Specialists. This position will serve as an integral member of the Health Services Team and will assist with the day-to-day activities of the Health Services Department. Responsible for coordinating the health and well-being of individuals. Ensuring compliance and implementation of quality nursing services to assigned individuals in accordance with Federal, State, and local standards. Responsibilities Performs and/or ensures all assessments are completed as identified in the Physical Health section of the policy manual. Participates in the admissions and discharge processes for individuals including but not limited to planning and evaluation. Participates in the development of individual planning ensuring all health needs are identified and addressed. Reviews physician and other clinician orders to verify accuracy and correct implementation. Assist with physical assessments of new employees, annual PPD's, vaccines, illnesses and injuries of employees when needed. Determines and participates in implementing appropriate action, treatment and response, follow-up care needed through resolution, via Nursing Care Plan as identified in policy and procedures. Attends meetings and ensures that communication is maintained with individuals, parents, concerned family and guardians as well all other service providers regarding all aspects of the individual. Keeps actively involved in the day-to-day operations of the department to anticipate potential problem areas and implement corrective action as needed. Maintains and Supervises adequate nursing levels to ensure safety and quality services are provided to individuals in accordance with policy and procedure. Ensures staff performance is reviewed identifying both strengths and areas of concerns using multiple tools including reviews and appropriate disciplinary action in accordance with policy. Completes and maintains all documentation regarding programming, health and safety, comprehensive assessments, including daily observation, log sheets, and daily reports. Schedules, teaches, or delegates staff in-services, new hire and ongoing trainings as assigned. Assist with the completion of all internal and external monitoring, including reports, action plans and corrective actions per established times. Serves as Acting Health Services Director in that person's absence. Administers medications and treatments as ordered. Monitors medication administration system to ensure continuity and quality. Monitors, documents, and reports individual responses to medical/nursing intervention. Ensures that all safety rules and procedures are followed by assigned personnel. Participates in Healthcare Administrator on Duty, unannounced off hour visits, and other duties as assigned for supervision of the facility. Attends and participates in interdisciplinary team meetings, in-services, and competency-based trainings. Ensures that all instances of abuse are reported and investigated as specified in the abuse policy and in accordance with the policy and procedure manual. Reports instances of abuse in accordance with specified guidelines in the abuse policy. Understands and adheres to all policies and procedures. Performs other duties as assigned. Required Skills and Abilities Ability to function as role model by maintaining and demonstrating professionalism. Ability to work in multiple settings with a diverse group of individuals. Must have compassion, patience, and ability to nurture. Must be able to read and write American English proficiently. Must maintain training competencies as assigned. Must possess good time management and organizational skills to prioritize work for timely completion in a fast-paced environment with minimal supervision. Knowledge of Center for Medicaid Services rules and regulations, Nurse Practice Act for assigned state, and applicable laws and regulations. Possession of a valid Driver's License with required endorsement, along with an acceptable driving record for the past three years is required to transport. Ability to understand, apply and demonstrate the process of data collection. Ability to understand, implement and monitor various software systems for training, record keeping and employee management. Education and Experience Valid and current Registered Nurse license to practice in the state employed Required. Minimum two years of nursing experience required. Minimum of one year's previous supervisory experience required. Experience working with developmentally disabled individuals preferred. Must be CPR Certified. Physical Requirements Ability to work in close physical contact for providing personal hygiene assistance to individuals. Must be physically capable of transferring and lifting persons with or without reasonable accommodation. Must be able to reach, bend, kneel and stand for periods of time. May experience prolonged periods of sitting at a desk and working on a computer.
