Non-Exempt, Full-Time (30 Hours) About Us Life Care Concierge of South Florida provides personalized, high-touch care management and coordination services for older adults and their families during periods of medical, emotional, and logistical complexity. We serve as a trusted partner to families navigating aging in place, hospital discharges, dementia-related changes, long-term care planning, and other significant life transitions. Our approach combines clinical expertise with compassion, discretion, and thoughtful advocacy. Through individualized care planning, seamless coordination, and ongoing support, we help clients and their families make informed decisions with greater clarity, confidence, and peace of mind. Position Overview The Nurse Care Manager is responsible for coordinating and overseeing individualized care for clients in their homes, with a primary focus on clinical oversight, care planning, and advocacy. This role ensures clients receive safe, appropriate, and high-quality care aligned with their medical needs, personal preferences, and long-term goals. The Nurse Care Manager serves as a liaison between clients, families, and healthcare providers, promoting continuity of care and supporting optimal client outcomes. This position is care management-focused and does not routinely provide hands-on personal care services (e.g., bathing, grooming, meal preparation), except in limited or emergency situations as appropriate. The Nurse Care Manager is expected to perform all duties in accordance with the established Lifecare Concierge of South Florida Care Management Standards and Protocols, ensuring adherence to clinical guidelines, documentation timelines, and communication standards.
CORE RESPONSIBILITIES
Care Assessment and Planning Conduct comprehensive initial assessments (medical, psychosocial, and environmental) using the approved template within 5 business days of the initial visit. Develop and implement individualized Plans of Care tailored to client diagnoses and long-term goals. Perform formal reassessments and update care plans as indicated by changes in clinical status or for long-term clients. Care Coordination and Oversight Coordinate services with healthcare providers, caregivers, and external agencies. Attend or facilitate medical appointments as needed and advocate on behalf of clients. Provide ongoing clinical oversight to ensure services align with the established Plan of Care. Health Monitoring and Documentation Monitor client condition and identify changes requiring intervention, escalation, or updates to the Plan of Care. Enter visit notes and billing data into Bill4Time (or designated system) daily to ensure reliability and accuracy. Submit all documentation and billing for the previous week no later than Tuesday of each week. Maintain accurate medication lists and provide updated copies to clients at each visit. Client and Family Communication Email the party responsible with a summary of the visit and next steps within 24 hours of each client contact. Confer routinely with the Principal/Administrator regarding medical concerns, status changes, or service terminations. Maintain professional, collaborative relationships with the client's interdisciplinary team, including legal, financial, and medical professionals. Safety and Risk Management Assess home environments and recommend modifications to support client safety and independence. Identify and mitigate risks, including fall prevention and medication management concerns. Professional Collaboration Collaborate with interdisciplinary team members to support comprehensive care delivery. Maintain professional relationships with healthcare, legal, financial, and community resources to support client needs. Additional Responsibilities Participate in team meetings, case reviews, and care planning discussions. Perform other duties as assigned to support client care and organizational objectives.
KNOWLEDGE, SKILLS, AND ABILITIES
Strong clinical judgment and the ability to exercise discretion in the performance of services. Excellent communication and interpersonal skills, with the ability to navigate sensitive family dynamics and care transitions. Professionalism in coordinating with legal, financial, and insurance disciplines. Proficiency in documentation, reporting, and care management software (e.g., Microsoft 365, Bill4Time). Ability to manage multiple clients and priorities effectively with minimal supervision. Ability to maintain confidentiality and handle sensitive client and family information with professionalism and discretion.
EDUCATION AND EXPERIENCE
Current and active Nursing License (RN or LPN) in the State of Florida required. Minimum of 3-5 years of experience in care management, home health, geriatrics, or a related clinical field.
SPECIAL REQUIREMENTS
Work is performed in clients' homes and community settings, which may include exposure to pets, allergens, and varying environmental conditions. Must have a valid driver's license, reliable transportation, and active automobile insurance for travel to client homes and medical appointments. Must provide advance email notification of planned absences, ensuring all client responsibilities are covered and scheduled appointments are shared with the Principal. Ability to stand, walk, and travel between locations; ability to lift or move items up to approximately 10 pounds. Ability to effectively communicate in person, by phone, and in writing with clients, families, and healthcare professionals. Ability to prioritize, problem-solve, and respond calmly in dynamic or urgent situations. Ability to accommodate occasional schedule changes or urgent client needs.
