Job Overview We are seeking a compassionate and dedicated RN/ Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. This role requires a strong understanding of patient needs, excellent communication skills, and the ability to work collaboratively with a multidisciplinary team.
Shifts Available:
7a - 7p 7p - 7a 3 shifts per week Duties Administer medications and treatments as prescribed by physicians. Monitor patients' vital signs and report any significant changes to the healthcare team. Assist with patient assessments, including physical examinations and medical history intake. Provide direct patient care, including wound care, catheterization, IV insertion, and tube feeding. Support patients with activities of daily living (ADLs) such as bathing, dressing, and mobility assistance. Maintain accurate medical documentation in compliance with HIPAA regulations. Collaborate with healthcare professionals to develop and implement individualized care plans. Educate patients and their families on health management and disease prevention strategies. Participate in discharge planning and case management activities as needed. Uphold infection control protocols and maintain a safe environment for patients. Qualifications Valid RN/ LPN license in the state of practice. Experience in critical care, long-term care, or assisted living preferred. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is advantageous. Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and CPT coding is beneficial. Experience working with diverse populations including geriatrics, pediatrics, or individuals with developmental disabilities is a plus. Strong skills in patient assessment, medication administration, phlebotomy, and basic life support (BLS). Ability to perform procedures such as venipuncture, suturing, or airway management as required by the role.
