Responsibilities Manage and supervise nursing staff, including RN case managers, ensuring adherence to clinical protocols and quality standards. Oversee patient assessments, care planning, and symptom management tailored to individual needs, including dementia care, Alzheimer's care, and palliative approaches. Coordinate interdisciplinary team activities involving social workers, chaplains, physicians, and therapists to deliver comprehensive hospice services. Ensure compliance with healthcare regulations such as

HIPAA, NCQA

standards, and Medicare/Medicaid requirements. Facilitate staff training on EMR systems like Epic or eClinicalWorks, medical documentation accuracy, ICD coding (ICD-9/ICD-10), and infection control procedures. Lead discharge planning efforts and collaborate with families on advanced care directives and end-of-life preferences. Monitor clinical outcomes through utilization review and management processes to optimize patient care quality and safety. Qualifications 2+ yrs RN Clinical Management experience in Home Hospice Valid RN license with current state registration; Bachelor's degree in Nursing or related healthcare field preferred. Extensive critical care experience including ICU or emergency medicine settings; hospital experience highly valued. Proven leadership skills with supervising or nurse management experience in hospice or long-term care environments. Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth; proficiency in medical documentation and ICD coding (ICD-9/ICD-10). Knowledge of physiology, anatomy, pharmacology related to pain management, ventilator use, IV infusion/insertion techniques, catheterization, and sterile processing. Experience working with individuals with disabilities or developmental disabilities; expertise in geriatrics or behavioral health is a plus. Strong understanding of patient assessment techniques including vital signs, physical examinations, tube feeding management, and medication administration. Excellent communication skills for triage calls, family coaching sessions, and interdisciplinary collaboration.
