Job Description

Sierra Drive Campus 1040 Sierra Dr Greenwood, Indiana 46143 The Ambulatory Care Manager III is key for working with assigned patient and/or beneficiaries, assessing, facilitating, planning, and advocating health needs on an individual basis. In this role you will assist the coordination of delivery of cost-effective Healthcare services and establish a transition plan for post-acute care. You will also meet established organizational goals as outlined in Franciscan Values and Service Standards and will have the ability to build positive relationships, affect change, perform critical analysis, plan and organize, and promote beneficiary and family autonomy.

WHAT YOU CAN EXPECT
Full Time Days Position
  • 8AM to 4:30PM, Monday to Friday, some weekends required.
Support transitions between levels of care with care coordination. Act as a liaison between patients, consulting physician, referring physician or outside agencies to coordinate patient appointments, admission, or surgery. Coordinate the exchange of information either written or verbal before and after patients are seen. Work collaboratively with other members of the Healthcare team in coordination of the patient's care. Meet patient and family to complete psychosocial history and assess family dynamics. Review computer record and chart and become familiar with the patient's history and current circumstances. Assess the Plan of Care to meet patient needs. Provide education for families, patients and other members of the healthcare team.

  • Required within 180 days of hire Certified Case Manager (CCM

  • Required 5 Years Nursing/Patient Care Experience
  • Required 1 Year Case Management or Care Coordination Experience
