Job Description

Cascadia of Boise is an exciting company who is rapidly growing and is currently hiring a talented and motivated Resident Care Manager (Unit Manager). Cascadia of Boise specializes in Rehab, Vent Care, and Long-Term Care. We are seeking a manager to oversee our LTC/Vent Unit! Responsibilities The primary purpose of the Resident Care Manager is to develop, evaluate, implement health care plans for individual patients, assist with management of individual medical plans, while providing the highest level of patient care. The Resident Care Manager also communicates treatment plans between families, doctors, providers and residents/patients. Evaluates updates resident/patient health care plans to achieve person-centered care. Conducts assessments for new residents/patients. Actively participates in clinical meetings. Uses SBAR to communicate patient's change of condition with physician. Reviews test results from medical exams. Implements physician orders and follow up with treatment plans. Communicates with families, health providers and patients. Receives consent, as required. Documents in the medical record to clearly reflect patient care & current medical condition. Supervises staff to successfully implement patient plans of care & provides excellent customer service. Solicits patient/resident feedback to understand their needs and the needs of the community. Advocates for raising the bar of patient/resident care within the facility and influences others to take actions that increase the quality of person-centered care. Displays responsibility by taking ownership of quality care. Shows dedication to enriching the lives of our patients/residents through empathy and compassion. Exhibits a commitment to results by looking for and recommending/implementing process for performance improvements. Maintains positivity while interacting with co-workers, patients/residents and guests at the facility. Performs other duties as assigned. Experience Six months experience in a long-term care environment preferred. Must have, as a minimum, one (1) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
Education:
Degree in Nursing from an accredited nursing school. RN or LPN required.
Licenses/Certification:
Current RN license in the state employed. Valid CPR certificate.
