Job Description

Nurse Unit Manager St. Patrick's Residence is a Faith-Based, nonprofit, Long-Term care, and Short-Term rehabilitation facility, which is served by the Carmelite Sisters for the Aged and Infirm. Since 1964, the Sisters and their staff and volunteers have lovingly cared for individuals with love, respect and dignity. We are seeking a Registered Nurse
  • RN•Unit Manager.
$90K to $95K annually based on circumstances including skills, qualifications, degrees and certifications, prior job experience, training, market data, and other relevant factors. Registered Nurse
  • RN Unit Manager Qualifications:
    Valid Illinois Nursing License as an RN
  • Registered Nurse. Minimum of 2 years of nursing supervisory experience. Minimum of 2 years of SNF and LTC experience. Have current CPR certification. Registered Nurse
RN Unit Manager Job Summary:
Review admissions and service requests for prospective admissions. Receive notification of admission and perform inpatient utilization review for length of stay/admission criteria. Request relevant clinical information and documents in operating system. Determine appropriate level of care post-discharge based on medical information available and criteria application. Assess and coordinate discharge planning needs with healthcare team members. Provide consultation for patients and families who are discharging home. Assist with facilitating discharge planning of patients and arrange needed post-acute discharge services. Post-follow up after patient returns to discharge destination.
