Healthcare Professionals accepted) Community Health-Nurse Manager - Nurse Supervisor—Signing Bonus! Oak Lawn, Illinois—Convenient to Chicago, IL We are seeking a Nurse Manager to provide oversight of nursing care to patients in a community health care setting. The Supervisor will support the RN team, attendance, assign tasks, ensure completion of tasks in a timely manner, and assist nursing support team in providing patient care as needed. Provide directions for patient flow, and assign staff appropriately. Assist staff with clinical services as needed. Ensure clinical areas are appropriately stocked with medical and office supplies. Function as the lead nurse in coordinating care coordination activities with the multidisciplinary team, patients, and other staff of the health care team with regards to the primary care management of quality assurance indicators, patient flow, promoting wellness and educational activities in the management of patient diseases|condition within a primary care setting. Promotes patient-centered practice; supervise and assist with tracking and monitoring patients outcomes through our patient registry and electronic medical records system templates and generate weekly and monthly reports on outcomes, assuring quality care by adhering to therapeutic standards; measuring health outcome against patient care goals and standards; making or recommending necessary adjustments. The hours are 8:00am to 4:30pm Monday through Friday and the maximum number of FTEs managed will be 10. Excellent opportunity to grow within the organization and a highly visible position. The base salary range it $75,000-$80,000+ depending upon experience with full benefits, including Medical, Dental, Vision and 401K with match and loan repayment eligibility.
Requirements:
RN Degree . BSN Requires effective communication skills Requires a minimum four (4) years' experience; preferably related to primary care, community health nursing, and|or ambulatory care nursing. Emphasis on primary care services (prevention, intervention, and maintenance|support). Requires a minimum (3) years' experience in supervision Demonstrated clinical skill in physical examination and nursing intervention Requires current CPR certification Commitment to interdisciplinary teamwork Requires a working knowledge of computers, keyboarding, Electronic Medical Records systems, and various programs (Excel, Word, PowerPoint, Outlook, etc. The Health Center is an
