This pivotal role involves developing, implementing, and overseeing strategies that promote physical, mental, and emotional well-being for residents and staff alike. This role is 2nd Shift (3pm-11pm). The ideal candidate will possess a passion for holistic care, exceptional leadership skills, and a deep understanding of various healthcare settings including assisted living, home health, hospice care, and acute care environments. As a key leader, you will foster a culture of health, safety, and compassion while ensuring compliance with all regulatory standards. RN or LPN License required in the state of Indiana. Duties Develop and execute strategic wellness programs tailored to the needs of residents across diverse care settings such as assisted living, home health, hospice, and acute care. Lead interdisciplinary teams by providing supervision, mentorship, and guidance to nursing staff, case managers, and wellness coordinators to ensure high-quality resident care. Oversee the creation and review of personalized care plans that incorporate physiology, anatomy knowledge, medical terminology, and resident preferences to optimize health outcomes. Manage documentation review processes including medical records, patient monitoring logs, and EMR (Electronic Medical Records) systems to ensure accuracy and compliance with HIPAA regulations. Coordinate shift management to ensure adequate staffing levels for all shifts while maintaining budgetary goals through effective resource allocation. Monitor resident progress through regular assessments, case management strategies, and documentation review to adapt care plans as needed. Ensure compliance with Medicare guidelines and other regulatory standards related to resident care documentation and billing procedures. Facilitate ongoing training on medical terminology, HIPAA compliance, medical records management, and emergency procedures for staff members. Lead efforts in budgeting for wellness programs by analyzing costs related to staffing, supplies, and resident services while striving for operational excellence. Experience Proven experience in critical care environments such as acute care hospitals or hospice settings with demonstrated expertise in managing complex patient needs. Extensive background in assisted living facilities or home health agencies with supervisory responsibilities overseeing nursing teams or care staff. Strong knowledge of physiology, anatomy, medical terminology, and case management principles essential for developing effective resident care plans. Demonstrated leadership skills with supervising experience in healthcare settings; ability to motivate teams toward excellence in resident-centered care. Familiarity with EMR systems for documentation review and patient monitoring; adept at ensuring compliance with HIPAA regulations. Experience managing budgets within healthcare organizations while balancing quality of care with financial sustainability. Knowledge of Medicare policies related to senior healthcare services; ability to navigate regulatory requirements confidently. Excellent communication skills to collaborate effectively with residents' families, staff members, healthcare providers, and external agencies.
