- Oversee daily patient care operations and clinical staff assignments
- Coordinate referrals, patient care needs, and care plan implementation
- Ensure timely patient admissions and compliance with regulatory standards
- Review and approve physician orders and plans of care
- Conduct OASIS reviews and maintain quality and compliance standards
- Coach, mentor, and support field clinicians
- Participate in interviewing and hiring clinical staff
- Monitor key branch performance metrics and partner with leadership on operational goals
- Coordinate communication among clinicians, physicians, patients, and families
- Support field visits as needed What You'll Need
- Associate degree in Nursing or higher required
- Active, unrestricted RN license in state of practice
- Minimum 2 years of Home Health or Hospice RN experience
- Previous supervisory or leadership experience preferred
- Homecare Homebase experience preferred
- Experience with Microsoft Office and ability to learn enterprise systems such as Workday
- Reliable transportation, valid driver's license, and current auto insurance
- Ability and willingness to travel within assigned territory
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment This is an opportunity to lead, mentor, and influence patient care while helping build a strong and successful clinical team.
