Job Summary:
The Registered Nurse (RN) is responsible for providing clinical oversight and supervision of personal care services within the Home Health Agency. This role ensures that all services are delivered in compliance with applicable state regulations, Medicaid requirements, and agency policies. The RN oversees care delivery, supervises personal care aides, manages participant care plans, and serves as a liaison with regulatory entities to ensure high-quality, safe, and effective care.
Key Responsibilities:
Ensure all personal care services are delivered in accordance with participant care plans, authorized units, and agency/Medicaid requirements Conduct routine (monthly) reviews of aide documentation to verify accuracy, completeness, and alignment with approved services Perform at least one annual on-site evaluation of each personal care aide to assess performance and adequacy of care Review, approve, and update participant care plans based on supervisory visits, aide reports, and clinical observations Ensure all care plan changes are properly documented and implemented • Communicate significant changes in participant condition to the appropriate state authority or designee and document accordingly Make timely recommendations regarding service increases, decreases, or terminations Participate in orientation and ongoing training of personal care aides to ensure competency and compliance Assign and oversee qualified trainers for on-the-job training of aides Participate in case conferences with regulatory representatives or designees to coordinate care and resolve concerns Ensure compliance with infection control standards and communicable disease reporting requirements Monitor adherence to participant rights, confidentiality standards, and agency grievance procedures Maintain complete, accurate, and audit-ready supervision and evaluation records for all personal care aides
Qualifications:
Active Registered Nurse (RN) license in the State of Indiana Minimum of two (2) years of nursing experience At least one (1) year of supervisory or administrative experience Experience in assisted living, hospice, or personal services agencies (PSA) does NOT count toward required supervisory/administrative experience Experience in home health care or Medicaid waiver programs Prior experience supervising direct care staff Knowledge of home health regulations and documentation standards

$50.00 - $60.00 per hour
