The Clinical Coordinator (LPN) is responsible for providing central clinical support for the field nurses. Responsibilities include but are not limited to communication with physicians for necessary orders; coordination of necessary lab and x-ray services; assistance with and ordering of medications; assistance with ordering of durable medical equipment; phone triage of patient issues in conjunction with the nursing team assigned to each patient; attend weekly IDT meetings, accurate documentation of care and timely submission of documentation in compliance with state and federal regulations.

Qualifications:

Active Licensed Practical Nursing in Louisiana At least 2 years of experience in healthcare. Preferred experience in palliative care or hospice. Computer experience including knowledge of Microsoft and electronic medical record keeping.
