Essential Responsibilities:

Provides each resident with professional nursing care, assessments and evaluations. Responsible for the management of nursing activities performed by Neighborhood/Household (NH/HH) staff ensuring that the highest degree of quality care is maintained at all times Works with a team of cross-trained staff to coordinate the functions of the NH/HH Has 24 hour responsibility for clinical functions of the NH/HH Qualifications A current, valid Registered nurse (LPN) license is required Minimum of one (1) year experience in long term care, prior MDS experience is required. Ability to effectively communicate, direct and delegate tasks. Other Qualifications Has not been excluded from participating in Medicare and Medicaid programs. Maintains eligibility to participate in Medicare and Medicaid programs throughout employment. Participate in pre-employment and annual Tuberculosis screening.
