Job Description

Job Summary:
The Population Health Nurse Supervisor will oversee the daily workflows of Techs and LPNs and focus on program development and effective leadership. The Nurse Supervisor's ability to establish optimal workflows and relationships among the team will be an integral part of this role. The Nurse Supervisor will demonstrate effective leadership skills in efforts to promote employee engagement, optimal performance, and professional growth. The Nurse Supervisor will also serve as a resource for the department on a wide variety of processes involved with scheduling and assisting with provider visits.
Essential Duties & Responsibilities:
  • Consistently operates with high-level of integrity and displays an excellent example of company culture, values, and policies
  • Provides direct supervision to the Tech/LPN team regarding daily workflows and productivity
  • Provides initial and ongoing education and training to the Tech/LPN team
  • Communicates effectively, objectively, and fairly with other staff regarding job role, responsibilities, efficiency, etc. as the needs arise
  • Clearly communicates productivity metrics and performance expectations
  • Continuously monitors productivity and performance of the Tech/LPN team and performs performance improvement plans, when necessary, with timely follow-up
  • Reviews and approves time & PTO requests
  • Performs Annual Performance Reviews and 90-day evaluations
  • Conducts interviews for Tech/LPN applicants and communicates correspondence to HR and Director
  • Ensures RNs obtain and maintain multi-state licensure.
  • Displays effective leadership skills that promote optimal employee performance and contribute to achieving short-term and long-term goals
  • Perform performance improvement plans for staff when necessary and provide timely follow-up
  • Reviews and approves time & PTO requests
  • Closely monitors time and attendance and assures protocols are followed
  • Performs annual performance reviews and 90-day evaluations
  • Develops new strategies when needed to assist with optimal program outcomes
  • Leads ongoing program development and tactics to achieve positive patient outcomes
  • Collaborates and communicates across multiple teams within the organization
  • Oversee implementation of new procedures and/or operational changes to support company vision and strategy
  • Provides open communication and engagement among employees by providing regularly scheduled team and 1:1 meetings
  • Reviews performance and productivity with employees on a regular basis and provides clear expectations and strategies for improvement when needed
  • Promotes professional growth with employees through delegation of additional tasks when appropriate
  • Collaborates with leadership team regarding staffing, weekly work assignments/groupings, and workflows
  • Offers strategies for improving workflows and resolving issues within the department as they arise
  • Implements workflow improvement strategies as needed and provides follow up to reassess effectiveness
  • Reviews applicant resumes, conduct interviews, and corresponds with Director and HR regarding hiring decisions based on approved departmental needs
  • Reviews, signs, and submits expense reports for the Tech/LPN team
  • Coordinates new employee onboarding and training for Techs/LPNs
  • Travels with providers as needed on home visits to assist with gathering patient history, reviewing medications, triaging, performing screenings, and EMR documentation
  • Assists with member outreach for scheduling provider visits
  • Accesses, collects, and transfers member information from multiple computer data systems
  • Assist with any new program initiatives that may arise.
Marginal Duties:
  • Provides support for supervision of other teams within the department as needed
  • Assists with special projects as needed
  • Other duties as assigned
Knowledge, Skills, and Abilities:
  • Minimum of 3 years of experience as a medical assistant or LPN required
  • Minimum of 2 years of proven leadership and people management experience required
  • Ability to lead people in a supportive manner that highlights individual strengths and continuously develops skills and abilities
  • Ability to delegate and prioritize tasks and responsibilities
  • Ability to work well under pressure in a deadline driven environment
  • Ability to navigate through multiple computer programs with ease including but not limited to Microsoft Office Suite (Word, Outlook, Excel, etc.)
  • Highly organized and efficient with strong attention to detail
  • Ability to multitask and prioritize in a fast-paced, ever-evolving environment
  • Excellent verbal and written communication skills
  • Ability to interact effectively with all levels of staff
Supervisory Responsibilities:
  • Includes, but not limited to, the supervisory duties described above
Supervision Received:
  • Works under direct supervision of department
Director Working Conditions:
  • This position operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, and photocopiers.
  • 40 hours per week, Monday-Friday 8:00 AM- 5:00 PM
  • Overtime may be required
  • Travel may be required and is reimbursed according to travel policy
  • Must maintain a valid, unencumbered driver's license

Pay:
$50,000.00 - $55,000.00 per year
