In this vital role, you will coordinate and optimize patient care in a compassionate and efficient manner. Join us in our mission to provide top-tier medical services to our community while engaging in meaningful work that directly impacts the lives of our patients. Develop, implement, and review personalized care plans for each patient. Coordinate with healthcare providers, patients, and families to ensure effective communication and delivery of care. Monitor patient health outcomes and modify care plans as needed. Provide guidance and education to patients and their families about health management and care options. Ensure compliance with healthcare regulations and standards. Participate in multidisciplinary teams to enhance patient care and outcomes. Strong clinical skills with a focus on geriatric care. Excellent communication and interpersonal skills. Proficient in electronic health record (EHR) systems. Ability to manage multiple patient cases simultaneously. Strong leadership and teamwork skills. Medical Insurance Dental Insurance Vision Insurance Life Insurance Valid registered nurse (RN) license in the state of Massachusetts. Certification in Case Management (CCM) preferred.
