Catholic Charities Maine Behavioral Health Network in Portland, Maine has an opening for a Full-Time Nurse Care Manager. The BHN Nurse Care Manager II is a Registered Nurse (RN) who serves as a supporting clinical member of the Behavioral Health Network team. This role provides comprehensive care management, clinical oversight, and consultation services across all Behavioral Health Network Programs. The NCM fulfills all responsibilities required under state, agency and program requirements with a primary specific assignment to MaineCare Section 92 Behavioral Health Home services while also supporting NCM supporting tasks across all Behavioral Health Network Programs. Qualifications:
Registered Nurse (RN), licensed by the Maine State Board of Nursing. License must be in good standing with the Maine State Board of Nursing. Minimum of four (4) years of direct service experience in a healthcare setting. At least two (2) years working with individuals with Severe and Persistent Mental Illness (SPMI), Serious Emotional Disturbance (SED), or co-occurring disorders. Experience with underserved populations and complex comorbidities preferred. Experience with electronic health records preferred.
