Ann Arbor, MI Job Details Full-time 1 day ago Qualifications RN License Full Job Description Essential Functions Works as part of a multidisciplinary team for both the Behavioral Health and MAT programs which includes MAT and psychiatric providers, and a peer recovery coach. Triages MAT and psychiatric patients ensuring appropriate and timely care. Coordinates between MAT and psychiatric provider(s) and pharmacy, obtains medication history, assists with prescription processing and refills, prior authorizations, insurance issues, use, safety, and storage. Facilitates patient appointments for medications for opioid use disorder and alcohol use disorder in conjunction with medical provider. Facilitates checking vitals, urine drug screen, MAPS Takes patient history Provides patient education on medication options, safer use strategies, use of naloxone for opioid overdose reversal Screens for other acute or chronic health issues Administers IM subcutaneous injections Develops plan of care in conjunction with medical provider In collaboration with the program staff: Collaborates with program manager to implement individualized care plans. Reviews care plans regularly/monthly with team and revises as appropriate. Ensures all elements of treatment programs including progress towards goals, medication dosage, toxicology screens, and other parameters are appropriately entered into the electronic medical record and that all data can be analyzed and reported. Communicates with medical providers, as well as outside agencies including detox, inpatient, hospitals, and emergency departments. Serves as liaison between providers, patients, families, collaborative service networks and other services Confers frequently with case manager and peer recovery coach to assess, monitor, and evaluate patients' rehabilitation and adherence to the program. Conducts outreach to patients who have not presented to care when expected. Ensures that all patients are seen at the appropriate intervals, in accordance with program protocols. Ensures that necessary parameters, such as no shows, referrals made, and referrals completed are tracked and reported as required. Participates in all components of the program and assists with the revision/modification of the program as it develops. Responds to patient questions, requests for advice, results of tests, and requests for refills per protocol. Coordinates care for patients, providing follow up and communication per provider request. Relays clinical information to and from the providers from patients, or personnel described in above item. Able to administer immunizations, and lab tests as needed. Complete telephone and clinical care documentation in EMR. Manages lab and test results, and long-acting injectable medications Education and Experience RN required Current Michigan licensure is required 2 years of experience as RN in outpatient, community mental health, psychiatry, or substance use setting preferred. Certifications, Licenses, and other Special Requirements RN required
