This role focuses on improving patient outcomes through proactive care coordination, chronic disease management, and patient-centered support for adult and pediatric patients. Key Responsibilities Identify and manage patient populations through referrals, risk stratification, and patient outreach Conduct assessments of medical, behavioral, social and educational needs Develop, implement, and monitor individualized care plans in collaboration with patients and interdisciplinary teams Provide patient education and self-management support Coordinate care across primary care, specialty care, hospital and community settings Complete post-hospital follow-up including medication reconciliation, discharge review, and appointment coordination Monitor patient progress and revise care plans as needed Support evidence-based interventions and preventive care initiatives Maintain accurate and timely documentation with the electronic health record (EHR) Participate in workflow improvement and Patient-Centered Medical Home (PCMH) initiatives Serve as a resource and liaison for patients, families, and care teams Provide operational oversight of the Complex Care Management Program, including supervision, support, training, and performance management of department staff to ensure high-quality patient care, workflow efficiency, regulatory compliance, and achievement of organizational goals. Qualifications Required Current Michigan Registered Nurse (RN) license Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Ability to work independently and collaboratively in a fast-paced healthcare environment Experience managing complex patient needs and coordinating care Proficiency with computers and electronic health records Preferred Three or more years of experience in primary care, ambulatory care, home health, hospital, or skilled nuring settings Previous care management or care coordination experience Experience with quality improvement initiatives Training in patient self-management support Skills & Competencies Care coordination and case management Patient advocacy and education Strong organizational and time management skills Ability to build positive patient relationships Adaptability and problem-solving skills Leadership and collaboration mindset
