Primary functions:

Interview residents, residents' representatives and others with information related to the Minimum Data Set (MDS) assessment. Collect all data necessary to complete MDS forms and related documentation. Make appropriate referrals to other health care professionals. Participate and/or conduct interdisciplinary meetings. Supervise and participate in the overall care of the residents Oversee clinical coordination, review labs, assist the physician with rounds Participate in the facilities Quality Assurance process to review facility systems as directed

Wage Information:

$44.45/hour -$48.95/hour based on experience as an

RN & LPN

•Including $0.85/hour MI pass through pay until 09.30.26 PLUS an additional $3.40/hour direct care pass through if working the floor Why join our team?
